Circumcision Mohel Patient care review the “5 P’s” of patient care refer to a widely used mnemonic framework for purposeful hourly rounding in nursing. It is designed to proactively address patient needs, improve safety, and enhance overall care satisfaction.

The 5 P’s stand for:

  • Pain: Assess the patient’s pain level on a standardized scale and determine if medication or other interventions are needed.
  • Position: Ensure the patient is comfortable and reposition them regularly to prevent skin breakdown, stiffness, and pressure injuries.
  • Potty / Personal Needs: Assist the patient with toileting, offer hydration, and help with personal hygiene needs.
  • Periphery: Make sure essential items (e.g., call bell, phone, water, glasses) are within the patient’s easy reach.
  • Pump: Check that intravenous (IV) pumps, monitors, and medical equipment are functioning correctly and set to the right parameters.
